This program is designed to monitor and provide comprehensive care for patients with chronic kidney disease. It includes scheduling follow-up appointments, identifying patients who are non-adherent to treatment, ordering laboratory tests, and providing specialized medical follow-up. Additionally, each patient’s kidney stage is assessed to determine whether they require care at a specialized chronic kidney disease facility.
